The Care Coordinator is a patient-focused role that helps successfully navigate the comprehensive care of our patients in their healthcare journey. This position delivers a delightful experience focused on empathy, clear communication and making things simple. The position is responsible for prioritizing and documenting administrative tasks in our internal tools and managing inbound/outbound calls. The position works closely to support our clinical team and strives to resolve all issues on behalf of the patient. This position reports to the Operations Manager and works collaboratively with the Director of Operations, Medical Director and clinicians.

Job Essentials

Appointment scheduling, notification, reminders, and troubleshooting
Receive inbound calls and place outbound calls from/to: patients, provider offices, insurance companies, labs and other individuals involved in our patients care
Prepare, organize, submit and manage care documentation in electronic medical records systems and other internal tools
Support our clinician team with a proactive mindset that includes prompt responses and high attention to detail
Support our patients health with empathy, clear communication, seamless transitions and simplifying steps where possible
